Supporting Population Health

The ultimate goal of our Physician Organization is to support our constituents in population health. Through utilizing care management and coordination, and with the assistance of a solid data analytics team, we aim to effectively and efficiently manage the health of patient populations. In providing value-based care, identifying social determinants of health, and appropriately identifying high-risk patients and intervening where needed, we can change the course of a population’s health.

A critical mechanism for success is the Population Health Services Organization (PHSO) – an optimized, physician-partnered operations entity that powers a system to improve health outcomes, promote economies of scale and coordination, and reduce variation across the continuum. A PHSO can sell its services to subscribers beyond the network and create new business opportunities for the system.  Participants might include smaller systems that can’t support robust population health management efforts on their own.
